Tubing annulus comunication in water injector wells

this well is a water injector well injecting treated sea water into the formation. it has been in operation for less than two years now.
My questions are,

1. what are some of the possible causes of Tubing annulus comunication in water injector wells in a deep sea operation.
2. what are some of the means for leak detection in such wells.
3.what are som of the best pratices when it comes to operating an injection well. Dos and Don'ts?
4. Any considerations to be looked when designing such a well.
