flash set of cement in setting tool

EXECUTIVE SUMMARY
The 4 "” liner Cementing operations was conducted and completed with no issues.
After the job was performed, the drill pipe string was pulled out 12 meters above the liner top
and the excess cement was circulated out of hole.
However when attempt to resume pulling out of whole, the pipe get stuck where several
attempt to pull out of hole was tried with no vain. Still circulation through the string is possible
(700 lpm @ 2400 - 2800 psi)
after pooh seeting tool +drill pipe the  setting tool ; was found bonded with the cement and  Wash out problem was observed at the drill string  connection .

I need any explanation .
