Hole packed off during 5" liner cementing job

RIH 5" liner successfully to bottom, circulated and picked cementing head. Tried to resume circulation, observed hole packed-off.
RIH 5" liner successfully to bottom, circulated and picked cementing head. Resume circulation, 3 bottoms up, pumped 8 bbl spacer (Fly) didnot shutt off pumps, just switched to spacer. After pumping 8 bbl of spacer, hole packed-off as the spacer was in the string.

What would be best solution to get rid of it?
