Tapered top and surface hole?

In environments where conductor has to be drilled and cemented (jetting not possible)

Has anyone experienced or knowledge of other operator's tapered hole and surface hole drilling techniques, methods, best practices.

i.e. A tapered Top and surface wellbore(s) are first drilled in one pass. Both the conductor and surface strings are then run integral as one, (e.g. On a prepacked LP and HP wellhead housing? i.e. made up as one unit )All run to surface casing wellbore depth and then cemented to surface in one cement job. End product resulting in a significant time saving.

Or any other areas, experiences from the past where this tapered top hole stratgey has been implemented, would be appreciated.
