Unsecured wearbushing in ESP tubing head (surface wellhead)

At a recent CWOP, concern was expressed about the unsecured wearbushing that will be installed in the ESP tubing bonnet that will be installed on the existing wellhead. The wearbushing is not secured in place

Prior to (intentionally) pullling the wearbushing, a lower completion will be run and then TCP "shoot and pull".

A perceived risk is that the wearbushing can be "unseated" by equipment passing through it and potentially recovered to surface.

1. Whether this has happened before.
2. What the consequences were
3. Any precautions that you took to avoid this happening
