setting liner hanger Packer ---- accidentally --

wish you are all okay ,

i would like to ask if anyone has experienced or witnessed an accidental satting of liner hanger Packer ,

the liner hanger i am talking about was set at only 28m (100 ft)
* no problem in casing (wiper trips were performed with no problems even scraping with no problems)
* pick up & make up 7" MECANIC LINER ASSEMBLY with no problems
* no weight was applied to the liner hanger
* no rotation was appllied

any additional data or explanation will be provided
